About Thrive
Thriving in the Brazos Valley and beyond
Locally-owned and CHAP accredited Thrive offers a variety of services to the home-bound patients in six counties -- and growing.
The Hernandez family has a long heritage with Texas A&M. With over 13 family members having graduated from Texas A&M and three currently enrolled, Carlos Hernandez was excited to open Thrive right here in the heart of Aggieland. Three years ago, Carlos first began by seeing patients for IV Therapy. Then, having worked with several local doctors and patients, Hernandez saw an area where he and his team could make a difference. Carlos then felt confident to provide Bryan-College Station (as well as five other counties) with a larger selection of home health services.
Thrive provides RNs, LVNs, Physical Therapists, Speech therapists, Occupational Therapists and medical social workers to perform a variety of services based on the needs of the patient. Services provided include, but are not limited to: Skilled Nursing, Wound Care, Patient & Family Education, Cardiac Pulmonary Care, Home Health Aid Services, Bowel and Bladder Program, Diabetic Teaching and Care, Anticoagulant Monitoring, Blood Pressure Monitoring, Medication Management, Infusion Therapies, Tracheotomy Care, Catheter Care, Ostomy Care, Central Line & Port Maintenance, Blood Sugar Assessment, Vitals Signs, Venipunctures and Medication Box Setup.
With all of the services provided by their professional staff, many believe that home health care is out of their budget. However, with the approval of the patient’s physician, Hernandez says that much of the costs associated with home health is covered by insurance. The base qualifications are that the patient must be home-bound, and must be approved for home care by their physician. Thrive will do the rest of the insurance approvals and let you know which services are covered, and then discuss the needs with the patient and family.
Staffing is an issue that is important to Hernandez. "We are fortunate to have been able to hand-pick our employees, which gives patients peace of mind, knowing they are in good hands with Thrive." All employees are licensed through the state of Texas and Thrive performs a thorough background check of all applicants. "We also do a conduct check once a year to ensure the safety of our patients," he added. Hernandez also noted that Thrive offers their staff continued education opportunities in order to better serve their clients.
Thrive is certified through the state of Texas and is one of only a few local home health agencies who is CHAP ( Community Health Accreditation Program) accredited. "We want to be the best at what we do, I knew that being CHAP accredited, would enable the physicians, as well as our patients to rest easy knowing they were in good hands with us." said Hernandez.
